  4) Name exactly as you want it to appear
  5) A shipping address.

Once we receive the email, please allow 24 hours to recieve an invoice with payment options/instructions. The invoice will arrive in a new email. It is possible for it to end up in a Spam folder, so please check there if you do not see it.
